Two things I forgot to post, or haven't seen posted recently:
The walkmesh in front of the cave in the Silverwood, just past that archway with all the skulls, is screwed up and you can't reach the cave (which may or may not have an actual cave beyond the entrance)
6c Northroad 1:
halfway up the left side of the map, in a culvert, if you mouse over the area your mouse changes from an arrow to a doorway, but whereever that door or cave entrance is, it's encased in solid rock.
Make that three things..
I don't know if anyone's reported this and had it fixed yet, but Ironshield's smithy sells these lovely Tower Shields +1, Eagle sun & Eye, described as "This is a standard Tower shield carried by a follower of Helm."

Recently my PC could not transition into the tavern in the Golden Oak in SM. Oddly, Dan's PC Garlus was able to use the transition. Despite clicking away my PC was stuck in the foyer. He could enter the atrium just fine though. Dan thought this was because I was a loser but I am hoping its a bug =p.
the silversmith in silverymoon has no return bin. and the silvered bastardsword that is sold there has a blade that does not connect to the handle leaving an ugly gap. now im stuck with the ugly sword and cant return it.
time to carve some faces with the ugly sword (replaces the ugly stick nicely).

6c High Hold: Interiors - Koldan does not give up his quest item. Both options the PC has start his "success" dialogue, but no roll is made and no item appears.
6b Highway 3 - A bunch of NPCs failed to transition/disappear at the eastern end of the map and are now engaged in some sort of mosh-pit may-dance.
6d Rivermoot: Interiors - the "Barrel" as sold by Salesk weighs one pound. His "Bucket" weighs 5½.
6d Rivermoot: Interiors - the merchant in Scolton's shop (I forgot her name) is currently missing.

Looking in my toolset, the silvered bastardsword I turned in for the silversmith appears fine in both human and halfling hands. Not sure where the gap in the sword came from, but it looks like it happened after I made it and turned it in. Weirdness to me....

I have noticed major differences -- mostly in color variations -- between custom tooled items as they appear in the toolset, and the items appearance in the TSM mod. I have discussed this with other players and they have noticed similar discrepancies. I bet this has something to do with the disappearing pieces of the bastard sword. There is also a bug that causes "gloves" that are tooled onto a set of armor to default to a "flesh" or ungloved appearance if you equip bracers. My custom tooled armor has "gloves" checked in the toolset and appears with black gloves in the toolset that matches the armor. When I equip my PC's bracers IG, the custom gloves disappear from view and it appears as if the PC is not wearing gloves at all (i.e. naked hands). I retooled the equipped bracers to color them "black" to approximate black gloves (since my rogue uh ... likes wearing gloves ... =p) and it is still the same result. Once equipped it defaults to the "flesh" tone ungloved look. Anyway, that was a tangent but maybe something similar is happening with the sword.
