I found a really big hammer, and beat "bug threads are bad, mmkay!" into everyone's heads. Also Hialmar added the bug reporting; its a drupal plugin.

Players who are struck multiple times within a flurry will be saved from death by the damage cap for that flurry, and will again be made killable during the next.
This one has a bug in it; it might let you die anyway, and may or may not send you to the morgue. However, it gives a distinct server message when it has fired: namely, it will tell you that you're bleeding badly and then kill you before you actually do any bleeding. Please take screenshots of this message in your server window if it happens to you.

If DMs need help verifying the veracity of the conditions, I can look at screenshots.

Players should screenshot as much as possible if there is any uncertainty. We can filter extra information down to needful information. Missing information is likely to rule in favor of "He's dead, Jim."
